Mediterranean diet
MEDAS points (0-14)
A food pattern: olive oil, vegetables, fruit, legumes, nuts, fish, and modest wine. Extra-virgin oil and mixed nuts are the PREDIMED supplements.
PREDIMED scored the pattern on a 14-item Mediterranean Diet Adherence Screener. Points go to olive oil as the main fat, daily fruit and vegetables, weekly legumes, fish, nuts, and sofrito, and to limits on red meat, butter, sweet drinks, and pastry. The pattern here is eaten without a calorie target. Energy-restricted Mediterranean advice plus exercise (PREDIMED-Plus) is a combined program. So is a Mediterranean weight-loss diet on top of a CPAP machine. Ultra-processed intake and an eating window are different diet clocks.

Mediterranean diet→Insulin resistance (HOMA-IR)
Higher Mediterranean diet decreases Insulin resistance (HOMA-IR).
In 418 older Spaniards free of diabetes, PREDIMED-Reus assigned a Mediterranean diet with free extra-virgin olive oil or mixed nuts against low-fat advice. After a median four years the pooled Mediterranean arms had about half the new type 2 diabetes (hazard ratio 0.48). The measure is diabetes incidence, not HOMA-IR.
