Berberine → Insulin resistance (HOMA-IR)

Higher Berberine decreases Insulin resistance (HOMA-IR).

Perez-Rubio 2014 found that 500 mg of berberine three times a day for three months raised the Matsuda insulin-sensitivity index from 2.1 to 3.1 versus placebo in 24 people with metabolic syndrome. Zhang 2008 lowered glucose versus placebo in 116 people with type 2 diabetes; the clamp contrast missed.

Mechanism

Authors treat berberine as an oral glucose-lowering alkaloid. They do not isolate one receptor. Perez-Rubio, Gonzalez-Ortiz, Martinez-Abundis and colleagues assigned 24 people with metabolic syndrome to 500 mg of berberine hydrochloride or placebo three times a day for three months. The Matsuda index rose from 2.1 to 3.1 (P<0.01). Glucose and insulin areas under the curve fell. Metabolic syndrome remitted in 36 percent (P=0.037). Zhang, Li, Zou and colleagues assigned 116 people with type 2 diabetes and dyslipidemia to 1.0 g of berberine a day or placebo for three months. Fasting and post-load glucose and HbA1c fell versus placebo (P<0.0001). Glucose disposal on a clamp rose after berberine (P=0.037) but not versus placebo (P=0.063). Ruiz-Herrera, Navarro-Lara, Andrade-Villanueva and colleagues assigned 43 adults with HIV, viral suppression, and metabolic syndrome to 500 mg three times a day or placebo for 20 weeks. 36 were analyzed. Weight, body-mass index, and insulin resistance fell.

Caveats

Researchers assigned people to berberine or a control, so this is a trial stamp. The insulin-sensitivity files are small. More papers of the same design do not settle the claim. Perez-Rubio is 24 people. Zhang's clamp missed versus placebo; the published win is glucose and lipids. Ruiz-Herrera is an HIV pilot. This is a journal entry, not medical advice and not a reason to take berberine for insulin resistance.

Effect

qualitative. Perez-Rubio 2014: 500 mg TID, 3 mo, N=24 MetS, Matsuda 2.1 to 3.1. Zhang 2008: 1.0 g/d, 3 mo, N=116 T2D, clamp vs placebo P=0.063. Ruiz-Herrera 2023: HIV MetS pilot, N=36.
