Higher Prolonged sitting increases Insulin resistance (HOMA-IR).
In 24 adults with type 2 diabetes, an 8-hour day of unbroken sitting produced a higher postprandial insulin curve than the same day with 3-minute walking or simple resistance breaks every half hour.

Mechanism
Dempsey sat 24 inactive adults with type 2 diabetes for 8 hours, or sat them and stood them up every 30 minutes for 3 minutes of light walking or simple resistance (half-squats, calf raises, gluteal contractions, knee raises). Meals were the same. Insulin incremental area fell from 3,293 to 2,104 (walking) and 2,066 (resistance) picomoles per hour per liter. Glucose incremental area fell from 24.2 to 14.8 and 14.7. Both P less than 0.001. Yates assigned 60 older South Asian and White European adults a 7.5-hour sit, the same sit plus 5 minutes of standing every 30 minutes, or 5 minutes of light walking. Walking lowered postprandial insulin 16.3 mU/L. The drop was larger in South Asians (22.4) than in White Europeans (10.3). Standing did not move insulin, glucose, or triglycerides. Wheeler added a 30-minute moderate walk to an 8-hour sit in 67 older adults with obesity. Exercise then sit cut insulin total area 18 percent. Exercise then 3-minute light walks every half hour cut it 25 percent. The extra 9 percent is the sitting-break increment on top of a training-like bout. People with higher HOMA2-IR dropped insulin more. A 2018 pool of three crossover days (62 people, including the 2017 type 2 sample) found the sitting-versus-breaks gap in glucose and insulin grew as fasting insulin and surrogate insulin resistance got worse. Muscle that is not asked to contract takes up less of a meal's glucose. The pancreas then puts out more insulin. Yates's standing-null says getting off the chair is not enough if the legs do not work.
Caveats
These are one-day laboratory assignments with standardized meals, not years of desk work. The stamp is the assigned sit-versus-break contrast. It is not a claim that a gym program is sitting inverted. Dempsey 2017 is 24 people and is available here only as an abstract. Mean age 62. Type 2 diabetes, not insulin-treated. Yates is 65 to 79. Wheeler is 67 on average, overweight to obese; 16 percent had impaired fasting glucose. Freire did not report insulin in the published abstract; it is here for the glucose area. Larsen is eight adults with type 1 diabetes on pumps. Glucose incremental area did not differ (P=0.086). Insulin incremental area was higher with breaks even though the pump was standardized. The authors read a better glucose-to-insulin ratio. That file does not flip the sign. The 2018 pool includes the 2017 type 2 sample. Those 24 people appear twice on this page.
